DVD Order Form Help

Each row is only for one tape. If you send more than 5 tapes, print more forms. You only need to fill the payment and mailing information on the first form.

Tape number: If you send more than one tape for one transfer job, mark your tapes with numbers. So we can find the corresponding tape.

Source Format: Select the format of the video tape(s) you are going to send us. For example: VHS, Hi-8, Video-8, or any video format on this planet.

Service Type: Select the package in the first row. Select the type for the additional time below for additional tapes.

DVD Title and Additional Time: We are going to make a nice finish on the DVD. So just name the movie, we will make it appear on the DVD menu screen, label, and case insert. If it's more than one disk but with the same content, we will give a volumn number. Type the title in the first row, then type the additional time (in minutes) below.

Unit Price: This is the rate.

Subtotal: Add up all the sums in this column.

Sales Tax: Is there anybody out there who doesn't know what sales tax is? Maybe not! But if you are living ouside of Massachusetts, then you don't have to pay. It's a strange law, isn't it?

Shipping:For our shipping rates, please refer to shipping rates.

Total: Total means adding everything up!

The rest of this form is quite straight forward.

When you provde creditcard information, the signature number is required. You can always find it at on the back of the card, after the card number. It's usually a 3 digit number.

If you send us check or money order, please do some simple math, because you will need to write the total.

If you pay us by credit card, you can either do the math yourself, if you like math, or leave it to us. But don't forget to include all other important information, and most importantly, sign the form.
